This session will highlight the important and unique aspects of caring for emerging adults with JIA from the perspectives of a young adult patient, a pediatric rheumatologist, and an adult rheumatologist. We will discuss the process of transition to adult care and evidence-based best practices in providing optimal care for patients before and after the transfer. We will highlight and discuss the medical needs of this patient population (e.g., JIA medications and birth control), and important considerations for disease characteristics particular to JIA (e.g., uveitis and TMJ involvement). We will also discuss the significant non-medical needs and concerns of this patient population, including education and support around at-risk behaviors (i.e., use of alcohol and drugs), mental-health issues, challenges associated with going to post-secondary school and/or moving away from home, and psychosocial stressors.
