This session will bring experts to share their experiences to: 1) identify key social risk factors that drive health disparities, 2) develop a roadmap to partner with health system administrators, social services, and policymakers to garner support for key services to address social risk factors in rheumatology-focused healthcare settings, and 3) implement and evaluate interventions that target adverse social barriers in rheumatology clinics and develop strategies to collaborate across diverse clinical practices, including low resource settings.
