This session will bring together experts who will share their experiences in: 1) identifying key social risk factors that contribute to the health of patients with rheumatic diseases. 2) developing a roadmap to partner with other disciplines, health system administrators, social services, and policymakers to screen for and address social risk factors in rheumatology-focused healthcare settings, and 3) developing successful interventions, with a focus on facilitators and barriers for screening and addressing social risk factor across different rheumatology practices and settings.
